If your student will need to keep medication at school, please bring the completed forms with the medication to the front office on Information Day, or email our school nurse for an appointment.  All medication, including over-the-counter medication, must be accompanied by a medication administration form, including parent and doctor signatures.  Prescription medication must have a pharmacy label that matches the information on the medication form.  Over-the-counter medication must arrive in a new, unopened container.
